The GLP-1 Exit Strategy

The GLP-1 Exit Strategy is a term coined by Dr. Shallanda Hunter, PharmD, MBA, RPh, CFNMP. It is the structured plan you build before you ever stop a GLP-1 medication: protecting muscle, metabolism, and habits so your results outlast your prescription.

I coined it because I kept watching the same story end the same way. Someone does everything right on the medication, then the prescription ends, and nobody handed them a plan for what comes next. The pen paused your appetite. It did not teach your metabolism anything. That second part is my job.

Why you need an exit strategy at all

Most people do not stay on GLP-1 therapy forever. Cost, insurance changes, side effects, supply issues, pregnancy planning, or simply reaching a goal. The off-ramp is coming for a lot of patients, planned or not.

The research on what happens next is sobering. In the STEP 1 trial extension, participants who stopped semaglutide regained roughly two thirds of their lost weight within a year. The SURMOUNT-4 trial showed a similar pattern after stopping tirzepatide, while those who continued kept losing. Regain is the default outcome of an unplanned exit.

Here is the part that matters: regain is not a willpower failure. Research from the STEP and SURMOUNT programs indicates a meaningful share of weight lost on GLP-1 therapy is lean soft tissue. Less muscle means a lower resting metabolic rate and less capacity to clear glucose. Add rebounding appetite signals, and biology is simply doing what biology does. A plan beats biology arguing with willpower. Every time.

The five parts of the GLP-1 Exit Strategy

One thing the GLP-1 Exit Strategy is not: a suggestion to stop your medication, or to stop it alone. Taper decisions belong to you and your prescriber, period. Many people appropriately stay on therapy long term. This is the education for everyone who will not, whether by choice or by circumstance.

When to start: day one, not month twelve

The best exit strategy begins the week you fill your first prescription. Muscle preservation, habit building, and lab tracking all work better with time. If you are already months in with no plan, the second-best day to start is today.
